Scaling Talbert House’s Successful Model of Integrated Care
Project: To open a third Hamilton County clinic, building on Talbert House’s proven model of integrating physical and behavioral healthcare, and to facilitate the development of a health information technology system to improve care coordination.

Project Description: Talbert House seeks to improve care and life expectancy for people who have behavioral health disorders and comorbid medical conditions by adding resources where people already go for care. With the help of bi3’s funds, Talbert House will build on its already proven approach by opening a third primary care clinic at the ADAS Center, where they offer multiple services. Integrated care services will include primary care, behavioral health, and care coordination. Funds will also support health information technology to improve care coordination and interoperability.
The ADAS Center annually serves 4,400 patients through its mental health and substance use disorder programs. Those programs include Talbert House’s Alcohol and Drug Addiction Partnership Treatment program (ADAPT); The Central Community Health Board of Hamilton County (CCHB), which contracts with Talbert House to provide the continuing care component of the ADAPT program at the ADAS Center; and The Crossroads Center, which operates a methadone medication-assisted treatment program, two residential programs for women, and an outpatient clinic at the ADAS Center.
Through its expansion, Talbert House will serve patients from all three agencies with goals of improving the health status of patients with hypertension and diabetes. The project will serve approximately 60 percent of ADAS Center’s patient population over two years, after which time, operations will be sustained through Medicaid and Comprehensive Primary Care payments.
The grant will also allow Talbert House to put into effect an interoperable EHR system, which will allow the electronic sharing of patient information between different healthcare providers, improving the ease and efficiency of care coordination.
