Health Care Access Now (HCAN), in partnership with TriHealth Good Samaritan Hospital, is working to transform the healthcare system for those who suffer from chronic disease, thanks to a $1.75 million, three-year grant from bi3.

HCAN and Good Samaritan are working to improve chronic disease management in a patient population that faces many challenges. They also are redesigning and enhancing the quality of resident education in team-based primary care and outpatient management of chronic medical conditions that drive up health care costs and lower quality of life for patients.
