PalliaCare Cincinnati

Project: Community-based palliative care
Grantee: TriHealth in partnership with Hospice of Cincinnati
Investment: $3.35 million
Year: 2017

A $3.35 million grant from bi3 over three years enables TriHealth and Hospice of Cincinnati to launch PalliaCare Cincinnati, the first health system-sponsored community-based palliative care program in the region. Once established, the program will provide relief from the symptoms, pain and stress for people with a serious illness who live at home. PalliaCare also anticipates reducing healthcare costs due to lower use of acute healthcare services.

The program also addresses social needs and supports care choices to help improve patients’ overall quality of life. This innovative approach utilizes teams of nurse practitioners and social workers that collaborate with the patient’s primary care physician.

PalliaCare is different from hospice care because there is no life expectancy limitation.
